Website and clinical service terms

Terms of Service

Effective date: July 29, 2026

Please read these Terms carefully. They govern use of the public website and online services. Separate informed-consent, financial, privacy, and program documents may also apply to clinical care.

These Terms of Service ("Terms") are an agreement between you and Med Grade Fit, PLLC d/b/a Unleashed Medical ("Unleashed Medical," "we," "us," or "our"). By accessing unleashedmedical.com, scheduling a consultation, creating an account, or using our online services, you agree to these Terms.

1. Eligibility and geographic limitations

You must be at least 18 years old and legally capable of entering into these Terms. Clinical services are available only when you are physically located in a jurisdiction where the treating provider is authorized to practice. Unless we expressly state otherwise, Unleashed Medical provides clinical services only to adults physically located in Florida at the time of the encounter.

You agree to provide accurate identity, contact, health, payment, and location information. We may require identity and location verification before providing care.

2. Website information and the clinician-patient relationship

Public website, social-media, article, video, advertisement, and educational content is general information and is not personalized medical advice. Viewing content, sending a message, completing a questionnaire, or scheduling a consultation does not by itself guarantee acceptance as a patient, create a clinician-patient relationship, establish a diagnosis, or guarantee a prescription.

A clinician-patient relationship begins only after an authorized clinician accepts you for care and completes the evaluation required by applicable law and professional standards. Personalized clinical instructions provided during care control over general website content.

Unleashed Medical is not a substitute for comprehensive primary care. You should maintain an appropriate relationship with a local primary-care clinician and obtain in-person examinations, urgent evaluation, preventive services, and specialty care when recommended.

3. Telehealth services and limitations

Telehealth may involve video, telephone, secure messaging, electronic records, remote review of laboratory results, and other technologies. Telehealth has limitations, including incomplete physical examination, technology failure, privacy risks, delayed communication, and circumstances requiring in-person care.

We may determine that telehealth is not appropriate, require an in-person examination, refer you to another clinician, or decline or discontinue services when clinically or legally appropriate. Separate telehealth informed consent may be required and will control if it conflicts with these general Terms.

4. Emergencies and response times

Unleashed Medical is not an emergency service and does not provide continuous monitoring. If you believe you have a medical emergency, severe or rapidly worsening symptoms, thoughts of harming yourself or another person, or another urgent safety concern, call 911 or go to the nearest emergency department.

Patients may send treatment-related messages at any time, but messages are not continuously monitored and immediate responses are not guaranteed. Direct call, text, and email access does not create 24-hour emergency coverage.

5. Clinical evaluation, prescriptions, laboratories, and pharmacies

All diagnoses, treatment recommendations, prescriptions, refills, dose changes, and discontinuations are based on the independent clinical judgment of the treating provider. Treatment and prescriptions are never guaranteed. We may require medical records, identity verification, laboratory testing, follow-up visits, in-person evaluation, specialist clearance, prescription-monitoring review, or other information before initiating or continuing treatment.

Medications may be prescribed for an FDA-approved use or, when clinically appropriate and legally permitted, for an off-label use. If a compounded medication is proposed, you may receive additional disclosures. Compounded medications are not FDA-approved and are not reviewed by the FDA for safety, effectiveness, or manufacturing quality before marketing.

Laboratories and pharmacies are independent licensed entities. Their availability, processing times, pricing, shipping, dispensing decisions, substitutions, and policies may be outside our control. A pharmacy may decline to fill a prescription. You are responsible for following pharmacy instructions and promptly reporting delivery, quality, or medication concerns.

6. Your responsibilities as a patient or user

You agree to:

  • Provide complete, truthful, and current information about symptoms, diagnoses, medications, supplements, allergies, substance use, laboratory results, and other care.
  • Disclose care received from other clinicians and avoid duplicate or conflicting prescriptions.
  • Use medication only as prescribed and never sell, transfer, share, alter, or misuse it.
  • Complete required laboratory testing and follow-up within requested timeframes.
  • Notify us of adverse effects, hospitalization, pregnancy, significant medical changes, or new medications.
  • Keep account credentials secure and promptly report suspected unauthorized access.
  • Maintain updated payment, shipping, email, telephone, and physical-location information.

We may pause or discontinue prescribing or services for safety concerns, nonpayment, repeated missed follow-up, failure to complete required monitoring, abusive conduct, suspected diversion or misuse, inaccurate information, or other clinically or legally appropriate reasons. When clinically required, we will provide reasonable information about transition or follow-up care.

7. Follow-up access

Active patients enrolled in a program that includes unlimited scheduled follow-up consultations may request follow-up as clinically appropriate and may contact the provider by call, text, or email for treatment-related questions. This benefit does not guarantee same-day appointments, immediate responses, 24-hour availability, or emergency care. Program-specific benefits and limits disclosed at enrollment control.

8. Fees, recurring charges, cancellation, and refunds

Current fees, included services, billing intervals, and any minimum commitment will be disclosed before you enroll or purchase. Fees may vary by service, testing, medication, pharmacy, shipping method, and individual clinical needs.

If you enroll in a recurring program, you authorize us or our payment processor to charge the payment method on file at the interval disclosed during enrollment until cancellation or termination. Automatic-renewal terms and cancellation instructions will be presented clearly at enrollment. For plans subject to renewal-notice requirements, we will provide legally required notice.

You may request cancellation by emailing admin@unleashedmedical.com or calling (352) 354-2391. Submit cancellation at least 48 hours before the next scheduled charge to reduce the risk that processing has already begun. Cancellation is prospective and does not eliminate amounts owed for completed services, laboratory orders, consultations, medication already ordered or dispensed, supplies, shipping, or other nonrecoverable costs.

Unless a separate written policy says otherwise, fees for completed consultations, laboratory orders, custom or patient-specific services, prescription processing, and medication already ordered, dispensed, or shipped are nonrefundable. If we determine before treatment begins that you are not eligible, any refund will be handled according to the financial terms disclosed when you paid. Chargebacks do not cancel contractual payment obligations.

9. Shipping and delivery

Estimated delivery dates are not guarantees. You are responsible for providing a secure and accurate delivery address, promptly retrieving temperature-sensitive packages, inspecting shipments, and following storage instructions. Report missing, damaged, incorrect, or temperature-compromised medication promptly. Replacement decisions depend on the facts, pharmacy policy, applicable law, and whether a controlled medication is involved.

10. Electronic communications

You consent to receive service-related communications by email, telephone, text message, portal message, or other methods you provide. Standard email and SMS may not be fully secure. We may redirect sensitive information to a secure method. Message and data rates may apply.

Marketing communications, if any, will be sent only as permitted by law. Consent to marketing messages is not a condition of receiving care. You may opt out of promotional email through the unsubscribe link and opt out of nonessential marketing texts by replying STOP. Opting out does not prevent legally permitted treatment, billing, safety, or administrative communications.

11. Privacy

Our Privacy Policy governs general website information. Our Notice of Privacy Practices governs PHI maintained in connection with care. Those documents are incorporated by reference.

12. Acceptable website use

You may use the website only for lawful personal purposes. You may not attempt unauthorized access, interfere with operation or security, introduce malicious code, scrape or copy protected content at scale, impersonate another person, submit false information, or use the site to violate law or another person's rights.

Website text, graphics, branding, design, photographs, videos, and other content are owned by or licensed to Unleashed Medical and are protected by intellectual-property law. We grant you a limited, revocable, nontransferable right to use the site for personal purposes. No other license is granted.

13. Third-party services and links

The site may link to independent scheduling, laboratory, pharmacy, payment, shipping, social-media, or other services. We do not control independent third parties and are not responsible for their separate terms, privacy practices, acts, omissions, availability, or content, except to the extent responsibility cannot legally be disclaimed.

14. Disclaimers and limitation of liability

The public website and general educational content are provided on an "as available" basis. We do not guarantee uninterrupted access, error-free operation, or that general content will be complete or current for every circumstance.

To the fullest extent allowed by law, Unleashed Medical is not liable for indirect, incidental, special, consequential, or punitive damages arising solely from use of or inability to use the public website. Nothing in these Terms limits rights or remedies that cannot legally be waived, and nothing is intended to disclaim liability for professional negligence, willful misconduct, or other liability that applicable law does not permit us to exclude.

15. Indemnification

You agree to be responsible for losses or claims arising from your unlawful misuse of the website, violation of these Terms, infringement of another person's rights, or fraudulent information, except to the extent caused by Unleashed Medical's own unlawful conduct.

16. Governing law and venue

These Terms are governed by Florida law, without regard to conflict-of-law rules. Unless applicable law requires otherwise, legal proceedings relating to the public website or these Terms must be brought in a court with jurisdiction in Marion County, Florida. This section does not change any rights or procedures that apply to professional-negligence or other health care claims under Florida law.

17. Changes, severability, and entire agreement

We may revise these Terms by posting an updated version with a new effective date. Material changes may be communicated by other reasonable means. Continued website use after the effective date means you accept the revised Terms; however, changes to patient financial or clinical obligations will be handled according to applicable law and any separate agreement.

If any provision is unenforceable, the remaining provisions remain in effect. These Terms, together with incorporated policies and any separate signed patient documents, form the agreement concerning the matters they address. If a separate signed clinical, financial, or informed-consent document conflicts with these general Terms, the more specific document controls for that subject.
