Important: This Privacy Policy explains how we collect and use information through our public website and related nonclinical services. Medical information maintained as part of your care is governed by our Notice of Privacy Practices.

This Privacy Policy describes how Med Grade Fit, PLLC d/b/a Unleashed Medical ("Unleashed Medical," "we," "us," or "our") collects, uses, discloses, and protects information when you visit unleashedmedical.com, schedule a consultation, communicate with us, or otherwise use our online services.

1. Scope and relationship to health information

This Policy applies to information collected through our public website, scheduling pages, general inquiries, advertising interactions, and other nonclinical online services. It does not replace our Notice of Privacy Practices, which explains how we use and disclose protected health information ("PHI") under the Health Insurance Portability and Accountability Act ("HIPAA") and other applicable health privacy laws.

Information you submit through a secure patient portal, medical intake form, telehealth platform, laboratory process, pharmacy process, or direct clinical communication may become part of your medical record and will be handled under our Notice of Privacy Practices and applicable law.

2. Information we collect

Information you provide

  • Contact and identity information: name, email address, telephone number, mailing address, date of birth, and other information used to identify or contact you.
  • Scheduling information: appointment requests, availability, consultation type, and related information.
  • Communications: information contained in emails, text messages, telephone calls, reviews, surveys, and other communications with us.
  • Transaction information: purchase or enrollment details and limited billing information. Payment-card information is generally processed by third-party payment processors rather than stored directly by us.
  • Health-related information: symptoms, medical history, laboratory information, treatment information, and other information you voluntarily provide in connection with care. When maintained by us as PHI, this information is governed by our Notice of Privacy Practices.

Information collected automatically

  • Internet Protocol address, browser type, operating system, device type, referral source, pages viewed, links clicked, and approximate location derived from an IP address.
  • Cookie identifiers and similar technical information used to operate, secure, measure, and improve the website.

3. How we use information

We may use information to:

  • Operate, maintain, secure, and improve our website and services.
  • Respond to questions and schedule consultations.
  • Verify identity, eligibility, and location.
  • Provide telehealth and related health care services when a patient relationship has been established.
  • Process payments, coordinate laboratory services, arrange prescription fulfillment, and provide customer support.
  • Send service-related notices, appointment reminders, policy updates, and other administrative communications.
  • Send marketing communications when permitted by law and consistent with your choices.
  • Detect fraud, misuse, security incidents, and legal or regulatory violations.
  • Comply with legal obligations and enforce our agreements.

We do not use information submitted through clinical intake, scheduling, patient portal, or treatment communications for targeted advertising.

4. How we disclose information

We do not sell or rent patient lists or PHI. We may disclose information as reasonably necessary to:

  • Service providers and business associates: website hosting, scheduling, secure communications, payment processing, data storage, information technology, analytics, shipping, customer support, and professional advisers.
  • Clinical partners: laboratories, licensed pharmacies, health care professionals, and other persons involved in treatment, payment, or health care operations, as permitted by law.
  • Legal and safety purposes: comply with law, court orders, subpoenas, licensing requirements, audits, investigations, or requests from government authorities; protect rights and safety; and prevent fraud or misuse.
  • Business transactions: evaluate or complete a merger, acquisition, financing, reorganization, or sale of assets, subject to applicable privacy and confidentiality requirements.
  • With your direction or consent: when you ask or authorize us to share information.

Vendors that handle PHI on our behalf are required to protect it as required by applicable law and, when required, a business associate agreement.

5. Cookies, analytics, and advertising technologies

Our website may use cookies and similar technologies needed for basic operation, security, preferences, traffic measurement, and performance. We may use analytics services to understand aggregate website use. We do not authorize analytics or advertising tools to use PHI submitted through secure clinical pages for advertising.

You can control many cookies through your browser settings. Disabling cookies may limit some website functions. Where applicable law requires consent or an opt-out mechanism, we will provide it.

6. Email, telephone, and text messages

When you provide contact information, we may use it for consultation scheduling, appointment reminders, treatment-related communication, account administration, and other service messages. Standard email and SMS text messaging may not be fully secure. Do not use general website forms, ordinary email, or SMS for emergencies. We may redirect sensitive matters to a secure communication method.

By providing a mobile number, you consent to service-related calls and text messages. Message and data rates may apply. Marketing texts, if any, require any consent required by law and are not a condition of purchasing services. You may opt out of nonessential marketing texts by replying STOP. Opting out does not prevent messages that are necessary for treatment, safety, billing, or legal compliance where permitted by law.

7. Security and retention

We use administrative, technical, and physical safeguards designed to protect information. No internet transmission, email, text message, or storage system can be guaranteed to be completely secure. If a breach occurs, we will provide notifications required by applicable law.

We retain information for as long as reasonably necessary for the purposes described in this Policy, to maintain medical and business records, resolve disputes, enforce agreements, and comply with legal, tax, licensing, and regulatory obligations.

8. Your choices and requests

  • You may unsubscribe from promotional email by using the unsubscribe link or contacting us.
  • You may request correction of general contact information by contacting us.
  • You may exercise rights concerning your medical record as explained in our Notice of Privacy Practices.
  • You may request access, correction, or deletion of other personal information where applicable law provides that right. Some information cannot be deleted because of medical-record, legal, billing, fraud-prevention, or regulatory requirements.

9. Age and geographic eligibility

Our clinical services are intended for adults age 18 or older who are physically located in Florida at the time services are provided. We do not knowingly offer clinical services to minors through this website.

10. Third-party sites

Our website may link to independent websites or services, including scheduling, laboratory, pharmacy, payment, and social-media services. Their privacy practices are governed by their own policies. We are not responsible for independent third-party websites.

11. Changes to this Policy

We may update this Policy to reflect changes in law, technology, vendors, or our practices. The revised version will be posted with a new effective date. Material changes may also be communicated by other appropriate means.
