This Privacy Policy describes how Med Grade Fit, PLLC d/b/a Unleashed Medical ("Unleashed Medical," "we," "us," or "our") collects, uses, discloses, and protects information when you visit our website, schedule a consultation, communicate with us, or otherwise use our online services.

This Policy applies to information collected through our public website, scheduling pages, lead forms, general inquiries, advertising interactions, and other nonclinical online services. It does not replace our Notice of Privacy Practices, which explains how we use and disclose protected health information ("PHI") under the Health Insurance Portability and Accountability Act ("HIPAA") and other applicable health privacy laws.
Information you submit through a secure patient portal, health intake form, virtual consultation platform, laboratory process, or direct clinical communication may become part of your medical record and will be handled under our Notice of Privacy Practices and applicable law.

Information Collected Automatically:
- Internet Protocol (IP) address, browser type, operating system, device type, referral source, pages viewed, links clicked, and approximate location derived from an IP address.

Service Providers & Business Associates: Website hosting, scheduling platforms, secure communication vendors, payment processors, data storage, analytics, customer support, and professional advisers.
Clinical Partners: Diagnostic laboratories, licensed health care professionals, and other persons involved in care operations as permitted by law.
Legal & Safety Purposes: Comply with law, court orders, subpoenas, licensing requirements, audits, or government requests; protect safety and rights; and prevent fraud.
Business Transactions: Evaluate or complete a merger, acquisition, reorganization, or sale of assets, subject to standard confidentiality requirements.
Vendors that handle PHI on our behalf are required to protect it as required by applicable law and a Business Associate Agreement (BAA).
5. Cookies, Analytics, and Advertising Technologies

We do not authorize analytics or advertising tools to use PHI submitted through secure clinical pages for advertising purposes.

6. Email, Telephone, and Text Messages
When you provide contact information, we may use it for consultation scheduling, appointment reminders, communication regarding services, account administration, and service messages. Standard email and SMS text messaging may not be fully secure; sensitive clinical matters should be communicated through secure direct channels.
By providing a mobile phone number, you consent to service-related calls and text messages. Message and data rates may apply. Marketing text messages, if any, require express consent and are never a condition of purchasing services. You may opt out of promotional marketing text messages at any time by replying STOP. Opting out does not prevent transactional messages necessary for appointment reminders, safety, billing, or legal compliance.
7. Security and Retention
We use administrative, technical, and physical safeguards designed to protect personal information. No internet transmission or storage system can be guaranteed to be completely secure. If a security breach occurs, we will provide notifications as required by law.
We retain information for as long as reasonably necessary for the purposes described in this Policy, to maintain business records, resolve disputes, enforce agreements, and comply with legal, tax, licensing, and regulatory obligations.

- Opt out of text communications by replying STOP.

Our consultation services are intended strictly for adults age 18 or older who are physically located in the state of Florida at the time services are provided. We do not knowingly collect information from or offer services to minors.
10. Third-Party Sites
Our website or ad forms may link to independent third-party services, including scheduling, laboratory, payment, or social media platforms. Their privacy practices are governed by their own respective policies, and we are not responsible for independent third-party websites.
11. Changes to This Policy
We may update this Policy to reflect changes in law, technology, vendors, or operational practices. The revised version will be posted with an updated Effective Date.
